The essential instrument settings are shown in Table 1. ‘Enzyme reagent was loaded as “2nd reagent,” and the modified color reagents A and B were “4th reagent” and “3rd reagent,” respectively. The instrument incubated sample and enzyme reagent for 4 mm in the inner chamber of the rotor. It then added 4-aminophenol to the inner chamber and sodium hydroxide to the outer chamber, mixed the contents of both chambers centrifugally, and read the absorbance against the reagent blank. Thisprocedure was entirely analogous to the manual procedure (2). Results by the adapted assay varied linearly with salicylate concentration up to 5 mmol/L. Between-run CVs of 2.5% and 1.5% were obtained at salicylate concentrations of 1.6 and 3.7 mmoLfL, respectively (n = 12). Mean analytical recovery of salicylate added to patients’ samples was 98.4% (SD 4.5%; n = 8). Analysis of 17 samples from suspected salicylate overdosepatients showed satisfactory correlations between the results obtained with the adapted method (y) and the methods of Trinder (3) (y = 0.94x 0.13; r = 0.997) and Keller (4) (y = 1.05x 0.01; r = 0.988). Reagent costper assay was reduced from #{163}1.84 with the original protocol (2) to #{163}0.25 with the adapted method, much closer to that of Trinder’s assay (0.11 per tube) (5). The adapted assay is rapid, accurate, and precise, and provides a cost-effective alternative to traditional methods of salicylate estimation.
